1. Referral
Initial contact with the Centre may be made by self, friend, family member, acute care facility, health care professional, insurance company or legal practitioner. An interview will be arranged as quickly as possible.
2. Assessment
The program components in which each person participates are based on an individualized treatment plan determined by a comprehensive assessment. This assessment process usually takes place within the first month the person is in the program.
3. Treatment
Following the assessment period, long-term goals are established. Length of stay in the program varies, based on a wide variety of factors, including severity of injury, number of services, individual progress, etc. Most people complete their program in about one year. Progress is measured at three-month intervals. Family members and other interested stakeholders are invited to participate at regular conferences.
4. Discharge
Discharge is a planned process and occurs when individual goals have been achieved. Consistent with the Centre’s outcome orientation, discharge planning begins at admission. Program supports are replaced gradually by normal and appropriate community resources.
5. Follow-Up
Monthly contact with each graduate continues for one-year post-discharge to ensure a smooth transition into the community. Follow-up services also provide a vehicle for evaluating effectiveness and efficiency of the BIRC program.
